Download the GP consent form and print it off. Once completed, upload the form where it says ‘Upload File’ on the homepage. Then complete the application form filling in as much information as possible. BASC members need to enter their BASC membership number in order to qualify for the discount
Fill in the additional form and complete your payment. We will then send the consent forms to your GP Practice requesting your medical records. This can take up to 28 days.
We will chase up the request with your GP after 14 days of the original request. If we have still not received the medical notes after 28 days we may contact you to ask you to help move things on with your GP.
Once we have your medical notes we aim to have the report to you within three working days so you can attach it to your application and send it to your police force. If you would like us to email the report to the police force directly then you must write down the email address on the application form and request it in the order notes.
